Full Job Description

Whiston Core 24 is a specialist liaison service that supports the rapid assessment of service user's mental health for those who present to the Emergency Department in times of crisis or, those who are referred from the acute general hospital wards and the formulation of treatment and risk management plans based on assessment of needs with the appropriate triage and diversion form the service.

The service operates on a shift system of long days (currently under review), inclusive of nights to provide 24/7 365 days a year service. The service has recently undergone Organisational Change to make the service more flexible across the Urgent Care footprint, providing support to Mersey Cares additional Core 24 services including Royal Liverpool, Warrington, Aintree and Southport Core 24 services where required., A unique opportunity has arisen within the Whiston Core 24 team based at Whiston Hospital for a band 6 Mental Health Practitioner. This role will involve working across the Emergency Department and the medical inpatient wards, providing assessment, care and treatment of service users with mental health issues secondary to physical health.

This role requires some knowledge of physical health comorbidities an how this impacts upon an individuals mental health in an acute hospital setting. The role involves working closely with colleagues in the acute trust, contributing within the MDTs and over seeing mental health care and treatment from admission to discharge.

We are looking for an experienced practitioner with a wealth of experience working in mental health services, inclusive of organic services.
