Memory Assessment Nurse for Diagnostic Services

+ To work as part of the multi disciplinary team to screen and triage appropriate referrals
+ To complete a through and specialist assessment of clinical presentation and symptoms
+ To share information amongst the multi disciplinary team and create appropriate and person centered care plans
+ To refer for appropriate diagnostic procedures eg scans, neuropsychology, occupational therapy
+ To prescribe treatments (if NMP)
+ To follow up on efficacy of medications
+ To complete accurate records, To screen referrals for appropriateness
To undertake initial assessments and create a treatment plan
To liaise with the MDT to explore symptoms and causes
To refer for additional assessments and diagnostic tests
To initiate treatment and complete follow up reviews

We are looking to recruit either a band 7 Non medical prescriber or a band 6 clinician to cover a years maternity leave within the service.
